This form is for volunteers of the Petawawa Minor Soccer Club to request reimbursement for costs that they have paid out of their own pocket. For example (but not limited to) coaching education costs and match officiating courses. 

Please note that uploaded items may be downloaded from this registration to be kept digitally for Petawawa Minor Soccer Club's future use. Uses such as, but not limited to:

• club tax purposes
• Eastern Ontario District Soccer Association verification and proof
• Ontario Soccer verification and proof
